Students should establish a supervisory committee as soon as possible and no later than the end of the student’s second semester in the program. The supervisory committee is comprised of at least four members of the University of Florida Graduate Faculty. At least two members, including the chair, must hold Graduate Faculty status in the Rehabilitation Science program. See faculty list for eligible members. One member serves as the external member and should be from a different educational discipline, with no ties to the home academic unit (cannot hold Graduate Faculty status in the Rehabilitation Science program). One regular member may be from the home academic unit or another unit.

This template is to serve as a general outline for information Rehabilitation Science PhD students should cover in meetings with their supervisory committees. Students are encouraged to customize the template to best suite their individual needs. The Rehabilitation Science Committee Meeting and Milestone Evaluation Form should also be completed to document all committee meetings.
Committee meeting and milestone evaluation form
The Rehabilitation Science Committee Meeting and Milestone Evaluation Form is required a minimum of four times during the student’s academic program. It should be completed following: 1) each committee meeting; 2) oral portion of qualifying exam; 3) oral portion of dissertation proposal; 4) dissertation defense. Form should be signed within 1 week of meeting/milestone.
