Undergraduate Research Volunteer Onboarding Instructions

The steps below must be completed before a student begins volunteer service.


Step 1: Submit Required Volunteer Documents

Required documents:

  1. Record of Volunteer Service Form

    • The supervisor must complete and sign Section 2.

    • The end date should cover the entire volunteer period through the end of the year.

    • If the end date expires sooner, a new form must be submitted each semester or whenever the end date passes.

  2. Emergency Contact Form

  3. HIPAA & Privacy General Awareness (PRV800) Certificate of Completion

  4. UF Health Security and Confidentiality Agreement

Student supervisors must upload these 4 documents to the

CHP HR Action Request Form

 

Step 2: Receive Your Letter of Acceptance

  • After the volunteer has been entered into the UF system, Bethany McClellon will provide a Letter of Acceptance to each volunteer.
  • This letter will be required when completing course registration for CLP 3911.

Step 3: (If Required) Register for CLP 3911 – Introduction to Clinical Research 

Students who wish to receive course credit for volunteer research work must register for CLP 3911 – Introduction to Clinical Research.

Registration Steps:

  1. Complete the CHP Enrollment Request Form.

  2. Upload the following two required documents when submitting the form:

    • Letter of Acceptance provided by Bethany

    • A brief statement from your research supervisor describing:

      • the research work you will be performing, and

      • their approval for you to register for X credits during the specified term

    This statement may be submitted as a screenshot or PDF of an email exchange.

Important:
If both documents are not uploaded, you will not be registered for the course.

Registration Deadline:

Students may only register for research credit during the designated university registration periods.

The final opportunity to register each semester is during the drop/add period.


Step 4: Additional Requirements for Volunteers with Patient Contact

If your role includes contact with patients, additional health screening is required.

  1. Contact Bethany McClellon.

  2. Bethany will submit the INOP form, which notifies the Student Health Care Center (SHCC) that a patient contact health assessment is required.

  3. After the INOP form is submitted, you will be notified when you can contact SHCC to schedule your appointment.

The department is not responsible for any volunteer health assessment charges.
